    INSTRUCTIONS FOR PREPARING THE LTA SWPPP:

    ; This LTA SWPPP is only for projects with land disturbances of less than one acre or no soil

    disturbance. For projects with land disturbances of an acre or greater refer to the SDCCD’s

    SWPPP template.

    ; The SWPPP preparer shall complete all yellow highlighted sections with project specific details.

    It is recommended that yellow highlighting be removed once the document is finalized and

    submitted for signature.

    ; Additonal instructions for completing the SWPPP are located throughout the document in blue

    text. All blue text should be deleted from the document once finalized and submitted for

    signature.

    ; The Title Page shall have the following information:

    o Project name;

    o Contractor ID number and name;

    o Date LTA SWPPP was prepared;

    o Signed and dated by David Backensto, Property Agent;

    o Fill in project name and location in the page header.

    ; The Project Information and Signature Page shall have the following information:

    o Project start date;

    o Contractor name, address, and phone number;

    o A detailed description of the work to be conducted, including size of project, demo

    work, construction, and any significant details about the project;

    o Project work location;

    o Name, signature, telephone number, fax number, and email address of the Qualified

    SWPPP Iimplementor (QSI) chosen for the project.

    o Name, signature, telephone number, fax number, and email address of the person

    who prepared the LTA SWPPP.

    ; The QSI shall have formal storm water management training as described under Training in the

    template.

    ; Attach a Location Map and Project Site Map for the project, with the required elements listed on

    page 2 of the template, to the LTA SWPPP.

    ; Complete Tables A-1 thru A-6 with the BMPs that will be used for the project site. Each of the

    BMPs with the dark hashed shading are required BMPs and shall be used for the project site. If

    these BMPs are not applicable due to site conditions, uncheck the block and explain why they

    were not selected. At least one of the BMPs with the vertical striped shading shall be used on

    site, at the descretion of the contractor based on site conditions and constraints. If the SWPPP

    preparer feels the need to designate one of these BMPs due to site conditions, the use of the

    BMP should be specified and the footnotes edited as appropriate. If these BMPs are not

    applicable, this should be indicated and the table edited as appropriate.

    All of the columns in a table associated with a BMP being used on site shall be completed for

    that BMP.

    ; Add additional site specific BMPs to be used for the project in the space provided at the end of

    each of the BMP sections in the template. This space may also be used to further discuss

    BMPs identified in the tables and their use for the project.

    2 LTA SWPPP

    ; Table A-7 should only be completed by the QSI in the event of any petroleum products or

    sewage release that enters a storm drain or receiving water.

    ; Attach the California Stormwater Quality Association (CASQA) Stormwater BMP

    Handbook Portal: Construction BMP fact sheets that correspond to the BMPs selected for

    the site. Avoid including all the BMPs as this LTA SWPPP should be tailored to the specific

    requirements of the project site. The CASQA BMP Factsheets are used based on the limited

    pre-permission copyright granted to the SDCCD SWPPP signatory for use in SWPPPs

    prepared for SDCCD facilities management projects. All other uses are prohibited and may be

    a violation of Federal Copyright Law.
