    eHealth4you: Tele-diagnosis of brain tumours

    Abstract

    eHealth4you provides clinicians with fast, efficient support, for non-invasive brain tumour

    classification using magnetic resonance spectroscopy (MRS). In addition to the morbidity and mortality implications of this non-invasive technique, the eHealth4you service typically implies a substantial cost savings, of at least one order of magnitude, when compared to traditional biopsies.

    We are looking for a commercial agreement with technical assistance or a licence agreement.

Background

     The diagnosis and treatment of brain tumours is typically based on a set of techniques that often need a histopathological diagnosis of a biopsy. Ongoing scientific research aims to change

    this situation using different approaches for the non-invasive classification of brain tumours. However, these methods face the crucial problem of collecting the biological information (metabolomic and genomic profiles) from several previous classified cases in order to create

    classifiers for each different type of tumour.

    eHealth4you provides is an evidence-based diagnosis service operated by trained clinical staff, based

    on a decision support system (DSS), which profits from automated and semi-automated pattern

    recognition strategies for single voxel (SV) human brain tumour 1H MRS spectra. This DSS contains

    validated spectral patterns at two echo times and anonymised clinical information from hundreds of

    validated human brain tumour patients and healthy controls. New 1H MRS data is automatically

    displayed in the GUI using pattern recognition algorithms to cluster included cases according to their

    pathology.

    Cases are processed from raw data by the automated modules for three major tumour super-classes:

    ; low-grade glial (grade two astrocytomas, low grade oligodendrogliomas; and low-grade

    oligoastrocytomas)

    ; benign meningiomas

Description of product / process

     eHealth4you provides clinicians at the subscribed organisations with evidence-based diagnosis reports for non-invasive brain tumour diagnosis using magnetic resonance spectroscopy. Anonymised MRS raw

    data travel encrypted through the Internet and is processed and analysed at the eHealth4you central facility,

    assuring no possible traceability to patients. This service complies with the highest medical ethics

    standards.

    The suggested type and grade of the brain tumour will be sent back to the submitting centre through the

    Internet jointly with a confidence estimate and a detailed clinical report elaborated by specialised

    eHealth4you diagnosis team. The clinician in the originating hospital/centre will always be responsible

    for the final diagnosis.

    What does the product or process do?

     The eHealth4you diagnosis team employs the DSS outcome, prior knowledge, and literature data to

    produce a spectroscopy-based orientation report with a confidence scale. The eHealth4you service

    can assist clinicians (neuro-radiologist, neuro-oncologist, neurosurgeon, and neurologists) in the

    diagnosis of a patient with a brain abnormality, i.e., a possible tumour.

    How does the product or process work? (without disclosing any confidential

    information)

     After obtaining a MRS scan of a patient the interested clinician logs into the eHealth4you web page

    and is instructed on the needed steps to anonymise the raw MRS data and submit it using a random

    identifier for tracking.

    eHealth4you clinical personnel processes this data and prepares a report with a suggested diagnosis

    for the brain abnormality. This report is encrypted and electronically sent to the referring clinician.

    Optionally at a later stage, the diagnosis can be improved by the eHealth4you clinical personnel, using

    MRI and clinical data from the referring centre. This two-phase methodology has proven to be

    successful at clinical centres in Catalonia.

    Under what conditions does the product or process work?

     The process requires access to MRS raw data, as obtained from the scanners from three

    leading vendors: Phillips, General Electric, and Siemens, and access to the Internet.

     End users must be clinicians from previously subscribed clinical centres, who will be authenticated to

    access the eHealth4you service. Hence, the only requirement at a subscribed institution is access to a

    MRS brain scanner and the Internet. MRS raw data will be strictly anonymised in situ for the use of

    eHealth4you, so no private patient information will be sent through the Internet.

    What are the applications of the product or process?

     Remote diagnosis of brain abnormalities with strong likeness of brain tumours at subscribed

    organisations.

    In addition to the morbidity and mortality implications of this non-invasive technique, the

    eHealth4you service typically implies a substantial saving of at least one order of magnitude

    when compared to traditional biopsies. Since the classification and clinical report is going to be

    provided in the framework of a tele-diagnosis service, each report will likely increase cost

    savings.

Innovative Aspects of the Technology / Production

    Brain tumours remain an important cause of morbidity and mortality and afflict an increasing

    percentage of the European population. In children over 1 year of age, brain tumours are the most

    common solid malignancies that cause disease-related death.

    Diagnosis using magnetic resonance imaging (MRI) is non-invasive, but only achieves 60-90%

    accuracy depending on the tumour type and grade. The current gold standard classification of a

    brain tumour by histopathological analysis of biopsy is an invasive surgical procedure and incurs a risk of 1-2% morbidity, in addition to healthcare costs and stress to patients. For tumours that evolve slowly (e.g. pilocytic astrocytoma in children), repeated biopsies may not be advisable or practical. There is a need to improve brain tumour classification, and to provide non-invasive methods for

    brain tumour diagnosis and prognosis, to aid patient management and treatment.

Main advantages of the Technology / Production

    Since the eHealth4you tele-diagnosis service only requires access to MRS raw data and the Internet, its cost entry point is affordable for institutions which may not otherwise have access to renowned classification and diagnosis techniques available only at leading centres across the globe.

    At this moment there is no other service similar to this one on the market. This is the starting

    point for enabling access to clinical centres, mainly in remote zones with difficult access, to a great deal of clinical and radiological diagnosis.

    HealthAgents

Abstract

    The HealthAgents network gives access to the results from hundreds of cases of brain tumours

    drawn from more than 50 hospitals across Europe, including the UK Children’s Cancer Study

    Group (UKCCSG) and several leading Spanish hospitals. All data is stored anonymously, and

    securely.

    Our proposal aims to enrich the HealthAgents network and therefore increase the case repository

    and the number of available classifiers. This network grants bona-fide access to any organisation

    in return for its contribution of clinical data to build the world largest distributed Data Warehouse

    (d-DWH).

    Diagnosis using magnetic resonance imaging (MRI) is non-invasive, but only achieves 60-90%

    accuracy depending on the tumour type and grade. The current gold standard classification of a brain tumour by histopathological analysis of biopsy is an invasive surgical procedure and incurs a risk of

    1-2% morbidity. By employing magnetic resonance spectroscopy (MRS) HealthAgents provides a non-invasive alternative for brain tumour diagnosis, to aid patient management and treatment, trying to decrease the risk of morbidity, the stress of the patients and also the cost of the classification. Then, its benefits are both financial and medical.

    Based on a multi-centre decision support system (DSS), HealthAgents makes available for any private or public hospital or diagnose centre the diagnosis and prognosis of tumours. Currently, cases are processed from raw data by the automated modules for three major tumour super-classes:
