The matrix Standard Assessment Process

Terms and Conditions/Charges

    Thank you for your interest in the matrix Standard. Completion of this Request for Assessment (RfA) is the point at which you are committing to an Assessment against the matrix Standard and as such you are subject to emqc

    Ltd’s standard Terms and Conditions and Cancellation Policy. To view these please visit our website

    The charge for an Assessment is ?600 per day plus expenses and VAT, for planning and preparation of the Assessment, on-site interviews, feedback and a written report. If you require a purchase order for an invoice this should be sent to us as soon as possible following the receipt of your estimated cost letter. We also operate a payment by instalments policy which may also assist you in spreading the cost of an Assessment. Completing this Request for Assessment Form

    The subsequent pages contain the Request for Assessment form (RfA) which should be completed and submitted electronically (along with additional information as described overleaf), to matrix@emqc.co.uk at least 6 weeks

    before your required Assessment date. This date we will endeavour to meet, however we cannot guarantee. This information is extremely important in helping us provide you with an accurate estimate of the cost of your Assessment. There is guidance on completing the form and understanding the Assessment process in “matrix

The Department for Business Innovation and Skills (BIS) owns the matrix Standard and as such, we are part of the

    MIAP (Managing Information Across Partners) scheme. In order for your Request for Assessment to be effectively processed, it is a requirement that a UKPRN number is obtained and provided in order for us to work effectively with our partner organisations. If you do not currently have a UKPRN number this can easily be gained by registering at www.ukrlp.co.uk

What Happens Next?

    On receipt of your RfA, we will acknowledge its receipt, estimate the number of days required to carry out your Assessment and allocate a Registered matrix Assessor to your organisation. The Assessor will then contact you

    to plan the Assessment and explain the Assessment process.

    Once complete, please send this RfA and all additional information electronically to matrix@emqc.co.uk. We will not be able to process your application unless all information is correctly completed and documents supplied as

    requested. All organisations should:

Complete Section 1 and Section 2

     E-mail a list of delivery people as recorded in section 3a (including name, job role and length of service)

     E-mail a full list of partner organisations (see section 2b for partner organisation description)

Then complete either: Section 3 or Section 4 or both
