    02. Basic Requirements

    A. Daily Objectives

The following task objectives are to be completed on a DAILY basis, Monday thru

    Friday, in all areas of the building. The contractor will provide all chemicals such as

    cleaners, wax, strippers, etc. and equipment such as mops, mop buckets, large

    garbage containers to remove trash from rooms, equipment, etc. to complete these

    tasks. This is the meaning of the statement hereinafter “The contractor will provide all chemicals and equipment to complete these tasks”.

     01: Clean Rest Rooms

     Clean/polish all stainless steel dispensers/disposal units

     Clean, disinfect and deodorize all commodes and urinals

     Clean all sinks and mirrors

     Sweep, mop and disinfect all floors

     Empty trash receptacles, including feminine napkins disposals; replace


     Refill all toilet paper, paper towel dispensers

     Clean walls, partitions and window ledges as needed.

     02: Floor Care

     Sweep and mop all tile floor areas including Conference Room

     Sweep and mop foyer area

     Vacuum rugs

    Scrape/remove gum, labels, scuff marks, etc from all floors including


     Spot clean carpet as needed

     03: Trash Removal

     Empty all trash cans and replace liners. Remove to outside dumpster

     Empty all “biohazard” bags and replace liners. Remove to Biohazard


     04: Clean Examination Rooms, Laboratory and Dental Operatory

     Clean sinks, counter tops and exam tables

     Polish sinks and chrome

     Refill Paper towels

     05. Glass

     Clean all glass and windows in each exterior and interior doors and front

     Desk window

     06. Clean Break Room & Conference Room

     01: Wipe off all tables and counter tops in break room area.

     Clean sink in Break Room

     Refill paper towels as needed

     02: Clean glass table top in Conference Room

     Dust all furniture including chairs

     07. Waiting Area

     Spot clean seats, furniture

    Wipe off all tables in waiting area with a damp cloth having cleaning

     solution on it

    Remove gum, stickers, etc from all furniture

    Check for and remove trash under tabletops and chairs

     08. Building Entranceways

     Sweep outside entranceways at front and back doors

     09. Check and report on lighting, inside and outside

     Check lights and advise of all defective lights including security lights on

     corners of building.

     10. Building Security

     Shut off all interior lights

     Close and lock all designated interior doors

     Ensure that all exterior doors are properly closed and locked

     Turn on exterior lights at each door

     Arm security system

     Notify police of any incidents or suspicious activities

    B. Weekly Objectives

    “The contractor will provide all chemicals and equipment to complete these


    01: Clean waiting area seats, furniture, and hand sanitizer dispenser with

    disinfectant solution as needed with disinfectant solution.

    02: Buff/polish all tile floor areas***

    ***Must have a qualified floor technician only (with at least five (5) years

    experience) to apply chemicals.

     03: Remove spots, stains and marks from cabinets, walls, door and door frames.

     04: Clean each door handle at least weekly with mild cleaning solution.

     05: Dust all furniture including conference room table and chairs.

C. Monthly Objectives

    “The contractor will provide all chemicals and equipment to complete these


    01: Dust all blinds and windowsills

    02: Clean all baseboards

D. Semiannual Objectives

    The contractor will provide all chemicals and equipment to complete all


    01: Light Fixtures: Clean Light lenses with mild cleaning solution and vacuum

    out fixtures. (Do not use water inside fixtures)

    02: Glass: Clean all windows (inside and outside) every Spring and Fall

    preferably after pollen settles in the Spring and about mid to late October in Fall.

    03: Carpets: Clean all carpeted floors using steam extraction method only (No

    rotary carpet cleaners.)

    04: Vinyl Tile: Strip/Refinish all vinyl floor areas*

    *Refinish must have two (2) coats of sealer, three (3) coats of finish @

    25% solids or four (4) coats @ 21% solids. Preferably in the months of

    March and August.

     05: Mini Blinds: Clean all mini blinds by washing with mild cleaning solution.

     06: Area Rugs: Clean all area rugs according to manufacturer’s instructions.

E. General Objectives

    01: Contractor will be responsible for reporting to management any repairs

    needing to me be made, i.e. damaged tiles, loose toilet seats, loose door knobs,

    commodes continuously running water.

F. Terms of Contract:

    Contractor agrees to provide the aforementioned services and duties for the sum

    of $____________ (annually). Payment will be made equaling


    G. Payment: Payments will be made upon completion of all work and acceptance by the

     Health Department representative within thirty (30) days of receipt of the invoice.

     Invoices are to be submitted to:

     Accounts Payable

    01: Itemize all invoices in full. Mail the original and one copy of your invoice to

    the address above. Each invoice is to include the following minimum information:

    1. Date invoice is submitted

    2. Payment terms

    3. Date of Service

    4. Service provided

    5. Due Date

    Invoices received with any of the required information listed above missing will

    not be considered as a “correct invoice”. All invoices submitted will be closely

    monitored for accuracy. Any invoice found incorrect will be returned to the vendor

    for correction.

    02: The Hart County Health Department is exempt from Federal Excise Tax and

    Georgia Sales and Use Tax. A certificate will be furnished if requested. The

    Health Department is exempt from taxes but the successful bidder will pay all

    taxes required of him/her by law and the Hart County Health Department cannon

    exempt others from tax.

    03: INQUIRIES REGARDING PAYMENT: All inquiries regarding payment of

    invoices are to be directed to Accounts Payable at (706)376-5117 or:

H. CANCELLATION WITHOUT CAUSE: Either party may end this contract without cause

    by giving the other party a minimum of thirty (30) days advance notice in writing.

I. ANTI-DISCRIMINATION CLAUSE: “The Hart County Health Department does not

    discriminate against any person because of race, color, religion, national origin, or handicap in employment or service provided”.

    J. COMMODITY STATUS: It is understood and agreed that materials used to deliver janitorial services to the Hart County Health Department will be new and in first quality condition.


    K. SCOPE: To provide for a basic purchase of Janitorial services for Hart County Health

    Department with a guarantee bid price for a twelve (12) month period. The

    contract will begin July 1, 2012 and be in effect until June 30, 2013.

    L. RENEWAL: The Hart County Health Department will bid the contract on a yearly basis.

M. BIO-HAZARDOUS MEDICAL WASTE: Occasionally, in the course of cleaning in a

    medical facility, the contractor and/or his/her staff may come into contact with bio-hazardous medial waste. It will be the responsibility of the contractor to provide training and ensure that their staff receives proper handling instructions for such occurrences. NOTE: A booklet called “What employees should know about Universal Precautions” is

    included with this packet.

    N. NUMBER OF STAFF ON SITE: Contractor will be expected to employ adequate staff to perform objectives listed above.

    O. AGE LIMIT: If Contractor utilizes any staff less than 20 years of age in performing the above objectives, it is expected that they will be under continuous supervision of an adult.

    P. COMMUNICATION: Contractor will assure that staff working at the Hart County Health Department is able to communicate with health department staff or will provide an interpreter for this purpose.


    1) To ensure payments are made in a timely manner for services rendered

    2) To ensure services are per contractual agreement

    3) To ensure contract is up to date on a yearly basis


    This Agreement will not automatically renew each year. Per State guidelines all

    contracts must be opened for bid on a yearly basis unless terminated earlier.

    Either party may terminate this Agreement by providing the other with a thirty (30)

    days prior written notice.


    1. The Parties to this Agreement shall treat all information that is obtained

    or viewed by it or through its staff and subcontractors performance under

    this Agreement as confidential information and shall not use any

    information so obtained, in any manner, except as may be necessary for

    the proper discharge of its obligations.

    2. The Parties to this Agreement also agree to comply with the Health

    Insurance Portability and Accountability Act of 1996 (“HIPAA”) and its

    amendment, rules, procedures, and regulations. The Parties to this

    Agreement acknowledges that HIPPA may require the Parties to this

    Agreement to sing a Business Associate Agreement or other documents

    for compliance purposes, including but not limited to a Business

    Associate Agreement.

    3. The Parties also agree to sign and comply with Attachment B.


    Except for the right of either party to apply to a court of competent jurisdiction for a temporary restraining order or other provisional remedy to preserve the status quo or prevent irreparable harm, the parties agree to attempt in good faith to promptly resolve and dispute, controversy or claim arising out of or relating to this Contract, including but not limited to payment disputes, through negotiations between senior management or the parties.


    All notices under this Contract shall be deemed duly given upon delivery, if delivered by hand, or three (3) calendar days after posting, if sent by registered or certified mail, return receipt requested, to a party hereto at the addresses set forth below or to such other address as a party may designate by notice pursuant hereto.

    No amendment, waiver, termination or discharge of this Contract, or any of the

    terms or provisions hereof, shall be binding upon either Party unless confirmed in

    writing. Nothing may be modified or amended, except by writing executed by

    both Parties.


    The Parties to this Agreement shall not assign this Contract, in whole or in part,

    without the prior written consent of the Georgia Department of Public Health, and

    any attempted assignment not in accordance herewith shall be null and void of

    no force or effect.


    Any section, subsection, paragraph, term condition, provision, or other part of this

    Contract that is judged, held, found or declared to be voidable, void, invalid,

    illegal or otherwise not fully enforceable shall not affect any other part of this

    Contract, and the remainder of this Contract shall continue to be of full force and

    effect as set out herein.


    This Contract constitutes the entire agreement between the Parties with respect to

    the subject matter hereof and supersedes all prior negotiations, representatives, or

    contract. No written or oral agreements, representatives, statements, negotiations,

    understandings, or discussions that are not set out, referenced, or specifically

    incorporated in this Contract shall in any way be binding or of effect between the
