I. PROJECT TITLE
Create a catchy title that sums up what your venture is about
II. PROJECT ABSTRACT
Describe your project in 100 words or less.
III. PROBLEM OR ISSUE STATEMENT
List down and briefly describe the problem(s) or issue(s) your project is attempting to solve or address.
Briefly narrate how your idea for the project proposal came about.
IV. PROJECT BENEFICIARIES
Who are the people and/or communities your project will help or provide service to?
V. YOUR SOLUTION
Briefly describe how your project (in general) will help solve the problem(s) or issue(s) you have listed.
Also explain in 5 – 7 sentences each of the following items:
a) The product(s) you will introduce or the service(s) you will provide
b) The need for the product/service you are providing
c) Your product/service’s main features (tell us what it’s all about)
*you can provide illustrations/pictures (attach as Appendix)
d) What is needed to produce your product/service? Briefly outline costs, time
requirements, future plans)
VI. YOUR OBJECTIVES
What would be the objectives of your venture, drawing from the solutions you mentioned above?
VII. WHAT YOU WANT TO ACHIEVE
What positive result(s) do you hope to bring to the people/communities you’ve identified after the implementation of your project?
Outline the activities you plan to do in order to achieve those positive result(s). How will you measure your success?
IX. LOGIC MODEL
Create a 1-page logic model for your project using the Kellogg Foundation Logic Model Development Guide as your template.

X. PROJECT TEAM
Who are your team members? What makes you the best people to implement this idea?
Do you have a mentor? Mentorship with outside experts is encouraged and allowed. Please identify mentors to your project and detail their involvement in the development of your idea.
XI. YOUR CUSTOMERS
Who are the people you intend to provide/sell your product/service to? Think of your ideal customer and describe how your product/service is going to appeal to them.
XII. YOUR COMPETITORS
Are there people offering the same product/service? What are their strengths and weaknesses?
What will your product/service have that they don’t? How will you make yours stand out from
theirs? How will you promote your product/service?
XIII. YOUR PROPOSED BUDGET
How will you use the P100,000 prize?
- key assumptions
- costs of operating your project (expenses)
- estimated income
If your venture is not-for-profit, how will you move from grants and donations to a self-sustaining organisation? (i.e. fee for your services, government support, volunteer labor)
XIV. RISK MANAGEMENT
What possible obstacles and risks will your venture encounter? What would be your contingency plan if these would ever to happen?
XV. SOCIAL RESPONSIBILITY
How will your venture take into account – or help bring about improvement – in any of the
- political - legal
- economic - environmental
- social - technological
i.e., Consider the balance of these factors in the implementation of your project – does it
reduce/add to an environmental issue? Promote/inhibit certain technologies? Boost/inhibit an economic activity? Include/isolate certain groups?
