Please accept this application/contract for exhibit space for the Arkansas School Nutrition thAssociation Annual Conference in Hot Springs, Arkansas to be held November 8th and 9 2013.

    I understand the following conditions for booth assignment and payment: Applications will be

    date stamped as received, with booth assignment priority given to those applications received first with complete payment. Booth assignments will be made upon receipt of no less than ? of the total cost of the booth. Final payment must be received no later than October 18th to hold booth assignment.

    This application is for exhibit space(s), as indicated below. Our check is enclosed for the amount of $______________. I understand that the cost of one booth includes: One 10’ x 10’

    booth with dividers and back, one sign, one table with cover, two chairs. I further understand that Visqueen floor covering is required for any booth featuring unpackaged food. I understand that it is my responsibility to arrange any electrical or other amenities through the convention center.

     This application/contract is subject to the Rules and Regulations on Sheet B

    No electrical outlet will be furnished. If you need electrical outlets, please let them know on the electric, phone & plumbing order that is sent to the convention center.

Special Notice:

    ; Brokers purchasing more than one booth should complete a FORM A for EACH

    company. Brokers who will have more than one line represented within a booth should

    complete Section 2 of each form. Distributors purchasing more than one booth under

    ONE company’s name should complete Section 1 of this form for the total number of

    booths purchased.

    should be submitted on the forms provided and sent directly to the address listed on the

    enclosed forms. Floor orders are more expensive; order early to avoid increased fees.
