    TMSETTING UP A PAYPULSE SURVEY

    Step 1 Complete Participation Form Distributed to Invitation List (see following page) 1. Survey Job Title Indicate the job title of the position.

    2. Survey Job Description Provide a brief job description (2-3 sentences) including primary tasks,

    responsibilities, and representative qualifications.

    Survey Job Tips:

     Job Titles: Avoid acronyms or firm-identifying titles. Include alternative job titles, if possible, for unique or potentially

    unclear job titles.

     Job Description: Use verbs that address key tasks and responsibilities. i.e., leads, develops, manages, oversees,

    supervises, analyzes, coordinates, etc. Include supervisory responsibilities and reporting relationship information to

    further clarify responsibilities and scope.

    3. Survey Date This is the date the survey is initiated (i.e., today’s date).

    4. Effective Date of Data This is automatically set to 90 days prior to the Survey Date. Participants are

    instructed to report their data as of this date.

    Note: This is done in compliance with anti-trust guidelines.

    5. Participation Deadline Indicate the date by which participants must respond to the survey.

    Participation Deadline Tip: 3 5 business days, or longer, provides for timely response and allows for schedules and

    availability of invited firms.

    6. Survey Items Indicate which items to survey by selecting the appropriate box(es) in the left column.

    a. Other Item: Up to two additional “non-complex” survey items are included in the $95 fee. Examples of non-

    complex survey items include:

    i. Offer sign-on bonus? Indicate amount.

    ii. Indicate firm job title.

     Additional charges may apply for “complex” questions such as differential pay by multiple shifts, explanation of

    specific pay practices, number of data cuts per job title, etc. Contact us for details.

    Step 2 Create Invitation List

    1. List the organizations you want to invite on page 3 of this Set Up Form. Be sure to include specific

    contact names and email addresses.

    Note: Organizations will be invited to participate in this survey via e-mail for speed and convenience purposes. This

    service DOES NOT include Denarius researching contact information. There is an additional fee of $15 per organization if

    you need Denarius’ assistance in obtaining contact information.

    Our Guarantee: If fewer than 5 organizations provide data, no report of results will be produced.

    However, your organization will not be charged a survey fee. If this occurs you may select additional

    organizations to invite and secure the necessary number of firms to generate a results report.

    Step 3 Submit Request

    1. After completing steps 1 and 2, email the form to Denarius (heather.lintner@denariusinc.com) to initiate

    the survey.

    2. Survey results will be distributed within 24 hours of the Participation Deadline. 3. Survey results will include a summary of the survey items selected as well as:

    a. Average Base

    b. Median Base ththc. 25 / 75 Percentile (minimum of 6 firms required)

    d. Average Range Minimum, Midpoint, Maximum (if selected)

    4. Survey results are distributed by email. Each participant also receives a copy.

    Instructions to Survey Sponsor: When setting up a survey, please indicate the organizations to invite,

    including a contact name and e-mail address.

     A minimum of five organizations must respond to your survey for a survey report to be generated.

    Therefore, build your list of survey participants anticipating that not all will have a job match or be

    available to respond.

     You must provide an e-mail address for each organization you want to invite participation.

    purposes.

     A list of all survey invitees will be included in the survey email invitation. A list of all survey invitees will
