    Participant & Parent Information (Sr.Age 4-H’ers)

I understand that pending funding and approvals, and potential team members, we would like to send a State 4-H Shotgun Team to represent Maryland at the National 4-H Shoot scheduled for June 20 25, 2011 in San Antonio, Texas. In addition to the event dates, there would be one or two days of travel to get to and from the event. There will also be numerous team practice dates prior to the event where 4-H’ers will have the opportunity

to demonstrate and improve their knowledge, skills, attitude and commitment. A State Team would then be selected from the state practice team participants. If selected, I understand that I would need to obtain funding to support part of my expenses for this National Trip either from personal sources, or through 4-H club and county sources. I am willing to commit to working toward and participating in this event if selected. As parent I am willing to support the 4-H’er in their participation in a national event.
