    Telephone Survey Script

    Protocol X665-09F

Q: HELLO

Hello, this is ________ calling on behalf of Drs. Jeff Todahl and Deborah Olson, researchers with the University of Oregon. We’d like to talk with adult survivors of interpersonal violence about trauma and healing. This is a difficult

and important topic. We will use what we learn from this survey to educate caregivers and service providers, and to raise awareness about trauma healing in our community. Your participation is anonymous and voluntary and takes about 14 minutes. We will not ask for your name or any identifying information. Some of the questions are sensitive

they will remind you about traumatic events you may have experienced and may make you feel uncomfortable. You

can skip any questions I ask if you’d like. Would you help us by your participation? Taking part in this interview is your agreement to participate. Do you have any questions?

If you have any questions or feel distressed about any topics we discuss, you may contact Dr. Todahl at (541) 346-0919. If you have any questions about your rights as a research subject, please contact the University of Oregon Office for the Protection for Human Subjects at (541) 346-2510.
