    1 Application Information

1.1 Overview

    Telecom New Zealand’s Permit to Connect Certification for WCDMA devices is 1.1.1 also known as a Telepermit.

    Applicants wishing to apply for the Telecom New Zealand Permit to Connect 1.1.2 Certification for WCDMA devices must complete this application form in full and

    send it to the Telecom New Zealand Device Technology Realisation (DTR)

    team. Once received DTR will contact you to discuss the relevant requirements

    for your submission.

1.2 Network Compatibility

    All devices submitted for Permit to Connect Certification must be compatible 1.2.1 with Telecom New Zealand WCDMA network and functional requirements and

    specifications. Details of minimum specifications are available upon request.

1.3 Regulatory Compliance

    All devices submitted for Permit to Connect Certification must comply with New 1.3.1 Zealand Health and Safety Standards, Radio Frequency (RF) performance

    parameters set by Telecom New Zealand and those imposed by New Zealand

    Ministry of Economic Development (MED).

1.4 Submission Requirements

    The applicant must provide various entrance requirements as specified in the 1.4.1 DTR102 WCDMA Device Certification Entrance Requirements document. This

    document is part of the WCDMA OEM pack and will be provided once this

    application has been accepted and a Non-Disclosure Agreement signed.

    All entrance requirements must be fulfilled before testing can commence or 1.4.2 Permit to Connect Certification is issued.

    All requirements defined in this document or any other document in the 1.4.3 WCDMA OEM pack must be provided by the applicant at no cost to Telecom

    New Zealand.

1.5 Permit to Connect Certification Testing Charges

    Charges may apply to testing and certification of WCDMA devices. All charges 1.5.1 are payable by the applicant in advance of testing and are regardless of

    whether or not Permit to Connect Certification is issued for the device

    submitted. Specific charges will be advised upon submission of this application.

    3 Declaration

In making this application, I undertake to comply with any additional conditions applicable to the grant of a Telecom New Zealand Permit to Connect

    Certificate should my application be successful. I agree to fully disclose the product specification, functionality and any defects

    prior to submitting this product.

    If this Permit to Connect application is successful I also agree to fully disclose

    any defects identified after the product has entered service.

I acknowledge that:

    ? All information, devices and test equipment supplied to Telecom New

    Zealand for the purposes of testing and approving the device are at no

    cost to Telecom New Zealand.

    ? Device testing will not commence until all appropriate charges are paid

    in full and submission requirements are satisfied.

    ? Completion of this application form in no way deems acceptance for

    testing or Telecom New Zealand’s approval of the product.

    ? Granting of a Permit to Connect Certificate is not an endorsement of

    the product and must not be used as such.

    ? Should for any reason this device no longer comply with the general

    and specific conditions of a Permit to Connect Certificate then Telecom

    New Zealand may withdraw that Certificate.

    ? In submitting this device all New Zealand regulatory requirements have

    been complied with.

If not the original manufacturer of the product, then I have the written

    approval of the Manufacturer to represent this product in the New Zealand
