     Submit loan to Countrywide via CWBC using Detailed 1003. If using DO, import DO findings into CWBC using Detailed

    1003. Note: AUS findings will only be honored if Detailed 1003 is used.

    another Lender.

     LDP/GSA Search Results for all Parties (not just the Borrowers)

     Initial 1003 signed & dated by borrowers (OR signed Borrower Authorization) and signed & dated by Loan Officer

     (Dated within three (3) business days of the date of application)

     Good Faith Estimate no more than 60 days old (Yield Spread Premium must be disclosed)

     All other borrower disclosures (Truth in Lending, FACTA, Servicing Disclosure, ECOA, Borrower Authorization, etc.)

     IRS Form 4506T signed by all Borrowers ? LandSafe Tri-Merge credit report OR DO credit report Credit for a spouse who is not a Borrower must be included for

     Uniform Residential Appraisal Report (URAR) and Invoice

     Homeowner’s Insurance (Countrywide Bank, FSB mortgagee clause)

     IF NEW CONSTRUCTION ONLY: Builder’s Certification, HUD-92541; Builder’s Warranty of Completion, HUD-92544; Evidence of

    10 Year Warranty Plan Coverage(if applicable); Inspection Reports HUD-92051 or HUD approved local building authority

    inspections (if applicable); Subterranean Termite Treatment Report-NPCA99a and NPCA99b

     Gaps in employment > 1 month
