    Graduate Student Health Insurance Policy

    Who is eligible:

    ? The Graduate Student Health Insurance Policy is a benefit for graduate students who are on a research (RA)

    or teaching (TA) assistantship or a University Fellowship (e.g. Graduate Fellowship, Thesis Completion

    Fellowship, Rasmuson Fellowship, NSF Fellowship, etc.).

    ? The University pays the policy on behalf of the student. Graduate students not on a research/teaching

    assistantship or fellowship CANNOT purchase this policy. They can purchase the “regular” UAF student

    health insurance policy during fee payment on line or through the UAF Business Office.

    How do students enroll in the Graduate Student Health Insurance Plan:

    ? Eligible graduate students are automatically enrolled by the Graduate School upon receipt of their

    assistantship/fellowship letter to the Graduate School.

    ? It is the students’ responsibility to provide a copy of their letter to the Graduate School each semester.

    ? Contract/Award letters must be delivered in person by the student and NOT faxed or sent to the Graduate

    School. At that time we will give each eligible student a copy of the Graduate Student Health Insurance

    Policy packet. We will make exceptions for students not physically in Fairbanks (but those on Westridge

    need to make the walk down to see us!).

    ? It is important for students to know that they are not to add the regular student insurance plan when

    they are completing registration online. They will not see an option for the Graduate Student Health

    Insurance Plan, as this is an automatic benefit for those eligible students.

    ?? Graduate students eligible for this policy cannot opt out of the Graduate Student Health Insurance even if they are

    covered by another insurance policy. If they are covered by another plan, remind them that double coverage is to

    their benefit. ??

? STUDENTS MUST HAVE SUBMITTED THEIR CONTRACT/AWARD LETTER TO THE

    GRADUATE SCHOOL BY THE LAST DAY OF FEE PAYMENT (September 12, 2008 for the fall

    semester) TO BE ELIGIBLE FOR THE INSURANCE FOR THAT SEMESTER.

    ? IF WE DO NOT HAVE A COPY OF THEIR LETTER BY THE END OF FEE PAYMENT THEY

    WILL NOT BE ENROLLED IN THE HEALTH INSURANCE PLAN FOR THAT SEMESTER.

    ? WE CANNOT ENROLL STUDENTS IN THE INSURANCE PLAN PAST THE LAST DAY OF

    FEE PAYMENT.

    ? ENCOURAGE STUDENTS NOT TO PROCRASTINATE!

    Policy period:

    ? Assistantship/fellowship for the fall semester only: student is covered for health insurance August 25, 2008

     January 11, 2009 regardless of the actual start date on their contract/award letter.

    ? Assistantship/fellowship for the spring semester only: student is covered January 12, 2009 August 24,

    2009. (Even if the student does not have a contract over the summer months, they are automatically

    enrolled for spring/summer coverage.) If a student starts their contract/award on January 6, 2009 (and was

    not covered by the Graduate Student Health Insurance Plan in the fall) their coverage would not start until

    January 12, 2009.

? Students that graduate in May, but have the spring/summer coverage will continue to be covered by the

    health insurance plan until August 24, 2009.

? Assistantship/fellowship for the entire year, student is covered August 25, 2008 August 24, 2009.

    Cost of the Plan:

    ? Remember students on RA’s, TA’s and fellowships do not pay for their coverage. Eligible students

    have the cost of the insurance premium paid by their individual funding source. Students on TA’s have the

    coverage paid for through special funds identified by the Provost. The benefit to the student and cost to the

    department or funding source is:

    Annual $1188.00

    Fall $ 442.00

    Spring/Summer $ 746.00

    Vision and Dental

    ? The plan does not cover vision and dental and coverage is not available to purchase separately.

    Deductible

    ? This plan has a $150.00 deductible for each injury or sickness per policy year to a maximum of $300.00.

    ? The deductible is waived for treatment rendered at the UAF Health Center. ? Students assigned to remote locations that do not have access to either UAF or UAA’s Health Center will

    have the deductible waived by special provider agreement. (There is an agreement in place that UAF

    students who have paid the UAF Health Center Fee, but live in the Anchorage area, can utilize the services

    at the UAA Health Center.) A list of identified special providers is in the policy brochure. In addition,

    Fisheries students assigned to Seward, Juneau or Kodiak, where there is no student health center, will not

    be charged a deductible when using services in their assigned communities. See page 6 of brochure.

    Dependents

    ? Eligible students may also insure their dependents. Eligible dependents are the spouse or Domestic Partner

    and unmarried children under the age of 19; or 23 if a full-time student. Dependent eligibility expires

    concurrently with that of the insured student. The cost for adding a dependent or spouse is as follows:

    NurseLine

    ? Insured students have access to nurse advice and health information 24 hours a day, 7 days a week by

    calling 1-800-883-2951. The NurseLine is staffed by registered nurses who can help students determine if

    they need to seek medical care, understand their medications or medical procedures, or learn ways to stay

    healthy.

    Global Emergency Services

    ? Insured students are automatically enrolled in the global emergency services from Scholastic Emergency

    Services (SES).

    ? This program connects students to doctors, hospitals, pharmacies, and certain other services when faced

    with a medical emergency while traveling 100 miles or more from your campus address or 100 miles away

    from your permanent home address.

    ? International students are eligible to receive SES services worldwide, except in the student’s home country.

    ? Be sure to check the guidelines on page 5 of the insurance brochure regarding access to these services.

    2008 Enrollment Form Graduate Student Plan

    ? The Enrollment form (which is included in the Health Insurance Packet and available online) is NOT

    to be filled out by the student unless they are going to insure dependents. Again, eligible students are

    automatically enrolled in the policy they do not fill out a separate form.

? A TEMPORARY UnitedHealthcare Insurance ID CARD is in the insurance information packet that

students pick up from the Graduate School. Students will receive a permanent card from UnitedHeathcare

after they are enrolled in the plan.
