Slingshot ’10-’11 NOMINATION FORM

We are thrilled release of the nominations form for the sixth edition of Slingshot: A

Resource Guide for Jewish Innovation. While it may seem like a lot of information to give, we believe this Form will help improve the quality of the nominations we receive,

give you the opportunity to portray a full picture of your project or organization to

evaluators, and result in a well-vetted book.

This form will stand as a complete submission for consideration in Slingshot ’10-‘11.

fundraising will not be considered unless the work of the organization is

innovative, effective, has an impact and strong leadership beyond the outreach

    programming.

matches the evaluation criteria (innovation, impact, leadership, organizational

effectiveness). If you are nominating an organization for which you work, please

give two additional references in the space provided such as board members,

funders, or clients who can serve as additional referrals.

multiple projects from the same organization will be competing against one

    another.

    6. In order to establish “organizational effectiveness,” each Nominee must submit

Nomination Form. Submissions missing the operating budget will not be

considered complete and will not be evaluated.

In order to be considered for Slingshot ’10-‘11, please send in your Nomination
