This form has two purposes:
1. The Head of School should use it to record his/her recommendation where the staff member is undertaking mid-term review or intending to apply for continuing appointment or promotion. Comments should cover performance at the University, and in the case of continuing appointment or promotion, they should be based on achievement discussed at the performance appraisals conducted in the intervening period.
2. The Head of School may use it to record his/her assessment of the staff member‟s achievements in the review period, summarised under the headings listed, in order to commend the staff member‟s achievements or to clarify any areas where an improvement in performance
is required. This should be done annually for staff on probationary appointments.
Where the Head of School has not been the „supervisor‟ at the Annual Review, he/she must sign this document after consultation with the
supervisor. If the Head of School does not accept the supervisor‟s recommendation, he/she must discuss this with the staff member.
