a. Request for appeal from Brian Humes re State Building Inspector and Office of

    Protection and Advocacy’s decision re W-19-09R re Court Street Fire Station,

    82 Court Street, Cromwell, CT A-48-09. Cross reference with appeal A-33-09.

    Michael Musco is recused. Panel consists of John Butkus, Panel Chair; Louis Free and

    *Robert Hanbury. There is no alternate. *Due to schedule conflict, Andrew Rizzo, Jr.

    replaced Robert Hanbury as appeal member. Hearing set for September 24, 2009, was

    cancelled; on August 20, 2009, appellant requested postponement. Hearing date set for

    Thursday, October 15, 2009 was postponed due to schedule conflict of panel member.

    Hearing was held on October 27, 2009, and recessed for additional evidence. Additional

    information should be submitted in writing by November 25, 2009. Hearing was continued

    decision re Meadow Ridge, 100 Redding Road, Redding, CT A-51-09. No panel

    was set. Refer to the Office of the State Fire Marshal (OSFM). On August 18, 2009, referred to

    the Office of the State Fire Marshal. As of October 14, 2009, no additional correspondence received.

    On October 14, 2009, Ignacy Kapalczynski, OSFM, advised that his office attempted contact with

    appellant approximately 3 weeks ago but have not heard from him. On December 1, 2009, certified

    letter sent to appellant requesting status of appeal and to respond in writing by Monday, December 14,

    2009. On December 14, 2009, correspondence received from appellant’s attorney stating

    they intend to continue to prosecute this appeal. On January 20, 2010, correspondence

    received from appellant’s attorney was forwarded to the Office of the State Fire Marshal. On

    February 9, 2010, correspondence received from Ignatius Kapatczynski, OSFM. On

    c. Request for appeal from Christine Bonito, Manager, re Meriden Fire Marshal’s

    decision re 3 Colony Street, Meriden, CT A-61-09. Andrew Rizzo, Jr. is recused. Panel consists of Michael Sinsigalli, Panel Chair; Thomas DiBlasi and

    Michael Musco. Marjorie Shansky will serve as alternate. On September 14, 2009,

    this was referred to the Office of the State Fire Marshal. On October 8, 2009, follow-

    up correspondence was received from Joseph Kingston, Office of the State Fire

    Marshal. Hearing set for November 16, 2009, was not held; on November 12, 2009,

    appellant requested postponement. Hearing was held on December 9, 2009. Decision

    to be sent out within 90 days [by March 9, 2010]. Decision was sent out on December 9, 2009. On December 14, 2009, correspondence received from Christine

    Bonito stating the hearing notice was not timely received by her. Proceeding to

    consider whether to reopen the appeal was held on January 27, 2010. On January 27, 2010, hearing was adjourned pending possible resolution. On March 12,

    2010, certified letter sent to appellant requesting status of appeal and to respond

    in writing by March 26, 2010. On March 26, 2010, correspondence received

    from appellant.

    d. Request for appeal from Thomas Andersen re State Building Inspector’s decision

    re M-875-09 re Northwest CT Sportsmen Association Clubhouse, Route 44,

    Colebrook, CT A-66-09. *Panel consists of George Iskra, Panel Chair; Andrew Rizzo,

    Jr., and Christopher Cozzi. Marjorie Shansky will be the alternate. *New panel was set on

    February 17, 2010, due to schedule conflicts: Andrew Rizzo, Jr., Panel Chair; Marjorie

    Shansky and Michael Sinsigalli. There is no alternate. On December 2, 2009,

    correspondence received from Lisa Humble, State Building Inspector, advising that Daniel

    Tierney will represent her at this appeal hearing. Hearing set for December 9, 2009 was

    postponed due to inclement weather. Hearing rescheduled to February 10, 2010, was

    again cancelled due to inclement weather. Hearing was held on February 24, 2010.

    Decision to be sent out within 90 days [by May 24, 2010]. Decision was sent out on

    March 16, 2010.

    e. Request for appeal from Frank Noe re State Building Inspector’s decision re

    M-1009-09 re Farmington Savings Bank, 427 West Avon Road, Avon, CT

    A-69-09. Henry Miga is recused. *Panel consists of George Iskra, Panel Chair;
